Requirements: Transition from Single to Married
- Member’s Change of Information Form (MCIF)
- Marriage Contract and 1 Photocopy provided by PSA or LCRO
- Valid ID and 1 Photocopy

To update your information, follow these steps:
- Write your 12-digit Pag-IBIG MID Number.
- Tick the "Change/Correction of Name" box.
- In the "To" section, write your maiden name, and in the "From" section, write your married name.
- Sign and date the form under "Certification."
- Submit the form to the Pag-IBIG office. Remember to get a priority number and wait for your turn to be called.
